Advance Health Care Directive
You have the right to give instructions about your own health care. You also have the right to name someone else to make health care decisions for you. An Advance Health Care Directive lets you do either or both of these things.

Uniform Statutory Power of Attorney
A Uniform Statutory Power of Attorney allows you to grant any or all of the following powers to an agent or agents or your choice: real property transactions; tangible personal property transactions; stock and bond transactions; commodity and option transactions; banking and other financial institution transactions; business operating transactions; insurance and annuity transactions; estate, trust, and other beneficiary transactions; claims and litigation; personal and family maintenance; benefits from social security, Medicare, Medicaid, or other government programs, or civil or military service, retirement plan transactions, and tax matters.
